A PLAN FOR DISPENSING MEDICATION - PRESCRIPTION AND NON-PRESCRIPTION
We will give your child prescription medication according to your written instruction. all medications must be in the original bottle dispensed by a pharmacist; with label containing the name of the child, the medicine type, dosage, how to administer the dose, frequency of, and the date on the prescription. Parent/Guardian is required to give written instruction, including time the medication is due. A daily medication log will be provided when you sign your chil in for the day.
Non-prescription medication requires both written physician and parent(s)/guardian(s) permission. If an authorized statement from the physician included medication, dosage, criteria for administration, and is signed and dated by the physician for an open span of time, it can be valid for one year. Parent(s)/Guardian(s) permission for this medication may also be for the open time span, as directed by the physician, but if the center uses the open permission, the center must notify the parent(s)/guardian(s) before the non-prescription medication is administered. When non-prescription medication is administered, the Director or Lead Teacher will sign the "Authorization for Medication" sheet. a separate medication log for non-prescription medication will be kept in the office or kitchen.

It is Cataumet Preschool & Child Care's policy that students and faculty wash their hands upon entering the classroom or beginning any food preparation.

We will apply sunscreen to students before going out for the day. Please remember to bring in a bottle for your child and sign the permission form. Both the form and sunscreen expire within one year. As we could be applying sunscreen on up to twenty children at a time, we are very appreciative of the sunscreen you can spray on. It is a huge time-saver!
